Length of treatment: The initial consultation
is 2 clinical hours. During this time, a treatment plan taking
each individual client’s short and long term needs
into account will be created. For all clients, this entails both a nutritional and psychological evaluation. For patients with diabetes mellitus, the initial evaluation also entails a comprehensive diabetes assessment. Most clients typically
meet with Dr. Mosunic once per week for 1 clinical hour, although additional
sessions may be scheduled if necessary. |

Treatment pricing: Each individual session (i.e., 1 clinical hour) is $185. The initial consultation (i.e., 2 clinical hours) is $370. Many of Dr. Mosunic's clients qualify for a psychological diagnosis and are sometimes covered by their insurance company's out-of-network benefits program. If a client does not receive any insurance reimbursement and is under financial strain, Dr. Mosunic may decrease his pricing based upon a sliding scale. Payment can be made via check, cash, Visa, Mastercard, or Discover. |

Session Length: With the exception of the initial consultation, individual appointments are 1 clinical hour in length and take place within the office or by special arrangement via telephone. Phone contact involving scheduling is not considered therapy and will never be billed. |

Emergencies: Dr. Mosunic will do his best to
accommodate emergencies on a case-by-case basis. |

Professional Referrals: Upon receiving a release of information from the potential client, please contact Dr. Mosunic to discuss the case. |

Speaking Engagements: Dr. Mosunic addresses both private and public audiences in the format of either a lecture, workshop or combination of the two. Topics include, but are not limited to, weight management, diabetes management, the relationship between diabetes and depression/anxiety, and improving cardiovascular health. Speaking engagements by Dr. Mosunic are typically done for a fee but are sometimes provided pro bono under special circumstances. Please call Dr. Mosunic for further information. |

Confidentiality & Discretion: As a psychologist practicing under the highest ethical standards of New York & Connecticut State law, all information that is disclosed within treatment with Dr. Mosunic is confidential and will not be revealed to anyone without the client’s written permission (or parental permission if under 18 years old). New York & Connecticut state law considers the relationship and communications between therapist and patient to be privileged and confidential.
The ethical standards of a licensed psychologist differentiates Dr. Mosunic from other weight loss professionals in that he is bound by law to keep his client’s information confidential. For example, Dr. Mosunic could not tell any person, (e.g., a close family member, the press, etc.) that the client is undergoing or has undergone treatment with Dr. Mosunic. The only time this ethical standard would be breached would be if Dr. Mosunic was told to do so by the client or the client is in danger of harming themselves or another person. Thus, the utmost discretion possible under law is followed.

Fee for Service: Dr. Mosunic is a fee for service
provider and is not part of any insurance company’s
network of providers. Services are rendered and charged to the individual client, not to your insurance company, adding an additional level of discretion. Upon request, a monthly statement will be provided to you to submit to your insurance company for reimbursement. This statement will include your diagnosis, the procedure code, the number of sessions, and the amount you have paid. Cost will differ depending on
each individual’s needs.
A written summary explaining Dr. Mosunic’s services
can also be provided to your insurance company upon
request. |
